I. ADMINISTRATION
Thank you for your interest in Clark School
of Massage. We understand your desire to seek a
new path toward earning a massage therapy license.
We are dedicated to your perseverance, commitment
and energies that drive you toward your goal of
personal and professional growth. The first goal of the
school is to prepare you to pass the Florida massage
license State Board Exam. However, we are equally

Ill. SCHOOL FACILITIES
The Clark School of Massage is located in Holiday, Florida, just off U.S. Highway 19, and North of Tarpon Springs.
The school is located at 4750 Mile Stretch Drive, Holiday, Florida.
The school consists of a reception area, lecture room with charts, skeleton and tables. A massage treatment area including massage tables, and a library for books, videos, and reading materials.
Future plans include a retail store, TV, and computer room.
lV. ADMINISTRATION POLICIES
NON-DISCRIMINATION POLICY
The Clark School of Massage, Inc. does not discriminate on the basis of race, color, religion, national origin, status as a disabled person, Veteran, age, gender, or sexual orientation.
ADMISSION REQUIREMENTS
Admission is based on the following criteria:
1. Must be 18 years of age.
2. Submit a high school diploma or GED equivalent.
3. Submit an official transcript from all high schools, colleges, or other academic institution previously attended.
4. Submit a written document containing a statement of how the student became interested in the field of Massage Therapy and their professional and personal goals.
5. Submit a signed enrollment agreement as required by the Commission for Independent Education.
6. Submit a picture with application.
7. Submit registration fee.
8. Have a personal or group interview with the administrators.
9. Submit a statement from your physician stating that you are in good health and physically capable of giving and receive massage.
ADMISSION PROCEDURES
Applicants should contact the school to schedule an appointment for a personal interview. If applicant decides to proceed the applicant should complete an application and present it with the registration fee.
V. STUDENT SERVICES
LIBRARY: The school has a resource library available for student use. This would include local and national journals on Massage Therapy.
HOUSING: This school does not provide housing or dorms for students enrolled at this school. Out of town students should arrive early to secure housing. Affordable housing may be found regularly listed in local newspapers such as the Tampa Tribune or the Saint Petersburg Times.
TRANSPORTATION: This school does not provide transportation for students enrolled at the school. The students must arrange for their own transportation to and from the school.
BOOKS: For the convenience of its students the school carries for purchase all text books needed for the courses offered at the school.
FINANCIAL ASSISTANCE: The Clark School of Massage does not accept or offer financial assistance.
CREDIT CARDS: Clark School of Massage accepts VISA and MasterCard only.
ADMISSION POLICIES: Due to the individual nature of instruction, enrollment is limited to 25 students per class. Students are asked to meet the following requirements and procedures:
1. Be at least 18 years of age.
2. Submit a copy of a high school diploma or GED equivalent.
3. Submit an application with a $75.00 registration fee, with a picture.
4. Schedule a pre-enrollment interview.
5. Submit a signed enrollment agreement as required by the Commission of Independent Education.
COUNSELING: The school administrators and faculty members are available to advise, assist and counsel students at their request regarding specific problems in their course work, techniques and work opportunities available in the field.
REFUND POLICY: The refund policy for all students is as follows:
1. Cancellation can be made in person, by electronic mail, by Certified Mail or by termination.
2. All monies will be refunded if the school does not accept the applicant or if the student cancels within three (3) business days after signing the enrollment agreement and making initial payment.
3. Cancellation after the third (3rd) business day, but before the first class, results in a refund of all monies paid, with the exception of the $75.00 registration fee.
4. Cancellation after attendance has begun, but prior to 40% completion of the program, will result in a Pro Rata refund computed on the number of hours completed to the total program hours.
5. Cancellation after completing 40% of the program will result in no refund.
6. Termination Date: In calculating the refund due to a student, the last date of actual attendance by the student is used in the calculation unless earlier written notice is received.
7. Refunds will be made within thirty (30) days of termination of student’s enrollment or receipt of Cancellation Notice from student.
PLACEMENT ASSISTANCE: While it does not guarantee employment, the school will provide notices of available positions sent to the school. We will keep statistical records of our students and their current working situation.
TRANSFER OF CREDITS: The school awards credit for previous academic work. The amount of such credit is determined on a case by case basis. An official transcript outlining prior training must be submitted with a school application and the before mentioned $75.00 fee. (Florida Board of Massage rules require transfer students to complete a minimum of 100 clock hours in a Board approved school.)
GRADING SYSTEM: Student progress in each course of the curriculum will be assessed by examinations given throughout the term. Weekly status will be given to the student regarding attendance and academic progress. Upon graduation the student will receive a copy of transcripts sent to the State Board of Massage. The grade received will be determined by the following scale:
A:90-100%
B:80-89%
C:70-79%
F: 0-69%
ACADEMIC PROBATION
Students must maintain a “C” average or better in all class work.
Students falling below a “C” level are placed on academic probation and counseled.
If grades do not improve within a four (4) week period, the student will be dismissed.
CONDITIONS FOR DISMISSAL
1. Failure to achieve the requirements of academic probation.
2. Attending classes under the influence of alcohol or drugs.
3. Failure to pay tuition.
4. Having three (3) unexcused absences within a one (1) month period.
5. Having four (4) days of unexcused lateness in a one (1) month period.
6. Behaving in any manner that may harm the reputation of the school, or that disrupts the academic pursuit.
CONDITIONS FOR REINSTATEMENT
Reinstatement following dismissal is at the discretion of the school director.
ATTENDANCE POLICY
Students are required to attend all classes as scheduled. any missed class must be made up in another scheduled class.
TARDY POLICY
Classes begin at the scheduled time. Students arriving after class begins are considered tardy. Time missed must be made up according to the make-up policy.
MAKE-UP POLICY
The student will have one (1) week to make up for a class day or portion thereof missed. It is the student’s responsibility to obtain all missed work from the teacher. All tests and/or quizzes must be made up on the next scheduled class day or as agreed to with the teacher.
LEAVE OF ABSENCE
A student may submit a request for a leave of absence up to one (1) month. If the student cannot make up missed work in a timely manner, the student may have to repeat the course at their expense. All arrangements must be pre-approved with the directors and will be handled on a case by case basis.
DISCLOSURE POLICY
Clark School of Massage will only disclose student educational information with a written consent from the student with the following exceptions:
1. To another school upon request, if the student intends and is seeking enrollment at that institution.
2. To officials of the Department of Education, the Inspector General, State and Local educational authorities, or other officials in connection with State or Federally supported education programs, with an official request.
3. To authorized parties in health or safety emergencies.
STUDENT RECORDS
Student records are kept on file for all students. The files are confidential and will not be released without a student’s written consent. Students can access their files with a written request. Viewing a file must be within school hours and within five (5) business days of request. Students requiring records to be sent to a third party must fill out an “Authorization to Release Student Records” form. There is a $2.00 charge for each copy of transcript and a $10.00 charge for each duplicate diploma.
VI. MASSAGE THERAPY PROGRAM
PROGRAM OBJECTIVES
The professional training program of the Clark School of Massage is designed to provide facilities, guidance, and environment for the systematic study of the art and science of Massage Therapy. Once completed the student will attain:
1. Eligibility to take the Florida State Exam.
2. Eligibility for National Certification.
3. Practical skills and confidence to perform as a Massage Therapist with emphasis on Swedish massage.
4. Certification in First Aid and CPR.
5. To give the graduate a through working knowledge of the structure and function of the human body and the theory of natural health care methodologies.

COURSE DESCRIPTION
CM101: MASSAGE THEORY/PRACTICE (225 HRS)
This course provides a comprehensive study of the history, theory and practice of Massage Therapy. The emphasis is on indications and contraindications of Massage and practical techniques of Massage applications and hygiene.
LAB: Hands on training in Massage techniques. This course demonstrates different techniques such as Effleurage, Friction, Tapotament. Also, alternative modalities as Myafascial Release, Trigger Point Therapy and Eastern modalities. The basic course focuses on Swedish Massage.
CM102: HYDROTHERAPY (15 HRS)
Discusses the scientific applications for water for the purposes of therapy and rehabilitation in its three states: liquid, solid, gas.
CM103: FLORIDA MASSAGE LAW (10 HRS)
The statutes, rules and history of Massage are discussed to familiarize students with principals governing the practice of Massage in the State of Florida.
Ethics, explores personal fears, prejudices and limitations and liabilities in the field of Massage.
CM104: ANATOMY/PHYSIOLOGY (150 HRS)
ANATOMY: The structure and functions are taught through directed readings, lectures, and hands on exercise. The student will explore the biology of cellular structure and the organization of tissues, organs and systems, as well as relevant pathologies.
PHYSIOLOGY: The study of the dynamics that occur within related body systems, including the nervous, musculo-skeletal and cardiovascular systems. The relationship between he body’s internal environment and homeostasis, which involves the constant monitoring of changes within and without, and the responses of the body’s inner mechanisms to these changes.
CM105: ALLIED MODALITIES (97 HRS)
Throughout the program the student is made aware of various Allied Modalities such as: Kinesiology Aroma Therapy, Seated Massage, Eastern Massage, Sports Massage, Mycology, Exercise and Stretching as they pertain to maintenance and rehabilitation.
CM106: HIV/AIDS EDUCATION (3 HRS)
This course fulfills the Florida State requirements for information and training on HIV/AIDS.
CM107: MEDICAL ERRORS PREVENTION
This is a required two hour course by the State of Florida. This class includes risk management, fire safety, and safety precautions necessary for health care professionals.
CM108: STUDENT CLINIC (40 HRS)
Student works on the public, and demonstrates competency is what they have learned.
Note:Each course is assigned a number with a pre-fix letter and are numbered according to teaching position in the calendar year.
VII. TUITION AND COSTS
TUITION: Tuition for Massage Therapy program, call school. Tuition charges do not include the $75.00 registration fee. A payment plan of three (3) quarterly payments is available with no interest or carrying charges.

Note:Tuition is subject to change. Costs agreed to on enrollment agreement will be honored.
BOOKS: The student will be charged $249.05 plus applicable sales tax for required textbooks. Price of books is subject to change without notice. Students are not required to purchase books from the school.
FIRST AID/CPR FEES: Certification is required by the Florida Massage Board. The course is given by an independent company. The fee will be paid directly to the instructor and is due at time course is given.
Vlll. CERTIFICATION AND LICENSURE
Florida requires a minimum of 500 hours massage training which is approved by the Board of Massage Therapy to prepare the student to be eligible to take the Florida State Board Exam. A massage therapy license is required to practice massage therapy in the state of Florida.
Students of this school will also be eligible for National Certification at the end of the course.
Licensing in other states may differ from Florida and it is the responsibility of the student to determine regulations thereof.
GRADUATION
Upon graduation, students will receive a certificate stating completion of the training program as required. The Certificate will not be awarded until all tuitions are paid. Students cannot sit for the Exam without the Completion Notification Certificate.
DRESS CODE
This school has no specific dress code. HOWEVER, students must present themselves in a decent, professional manner in keeping with the professional nature of the studies. Scrubs are acceptable attire and encouraged.
